Hi, I'm Nick.
A pharmacist
who builds things.

Asheville, NC · Mental Health · Healthtech · Psychedelics

I work at the edges of what healthcare is willing to try — building companies, networks, and care models with founders and executives who need someone who's already been there.

I entered this work believing people have an intrinsic capacity to heal. That medicine at its best supports that capacity rather than supplants it. Lowest dose. Shortest duration. Cross-disciplinary by design. Everything I build is in service of that orientation.

About

"The pattern is deliberate. I enter spaces before they are obvious and build the infrastructure that makes them work."

I entered compounding in 2011 when it was still considered the outcast corner of pharmacy. Most of my colleagues wanted nothing to do with it. I saw something worth fighting for: an integrative model of care designed around the patient, not the product catalog. That conviction turned out to be early. What was once fringe is now on the cover of every major health publication, prescribed by mainstream physicians, and driving some of the largest market movements in modern medicine.

From there: a national ketamine provider network with our own credentialing infrastructure, deep clinic relationships, and a distribution strategy that funneled patients through the employer and payor channel at a time when the infrastructure barely existed (and still barely does). A different project, same instinct: a cross-disciplinary community of 120+ organizations spanning clinicians, founders, researchers, policymakers, investors, and advocates convened around the psychedelic ecosystem before the field knew it needed one.

The pattern is not accidental. What connects all of it is a specific kind of fluency: the state and federal regulatory interplay that governs every space I have ever worked in. Compounding pharmacies exist in a permanent negotiation between state boards and FDA oversight. Psychedelics exist in the same tension, just with higher stakes and less precedent. I have been navigating that interplay since 2011 and it is the core of how I think about building anything in emerging healthcare.

My background spans emergency medicine, 503A compounding, digital health, and psychedelic-assisted care. I have operated at every layer of the stack: formulation chemistry and USP compliance, clinical protocol design, multi-state regulatory navigation, payor strategy, and closing the first B2B sale myself.

The thread running through all of it is a belief that a better model of care is possible. Not incremental improvement but a fundamentally different orientation. One that meets people before they are sick, that treats the whole person rather than the symptom, that reaches for the tools that actually work even when the system hasn't caught up yet. Integrative and preventative medicine are not fringe ideas. They are what medicine looks like when it takes seriously the possibility that a better world is within reach. That belief is what put me in compounding in 2011. It is what put me in ketamine clinics and psychedelic ecosystems. And it is what gets me out of bed to do this work now.

Outside of work I try to be present for the things that do not scale. Three kids. A partner. An edible garden in Asheville that asks nothing of me except that I show up.

Founded

Three companies built from zero — each a different layer of the healthcare system.

Meeko Health

Co-Founder & COO · 2023–2024

Ketamine-assisted therapy was producing remarkable clinical outcomes — but access was limited and no infrastructure existed to move it through the employer benefits channel. Working alongside co-founders who were first hires at Alto Pharmacy and had previously built billion-dollar companies, we built what became the first national ketamine clinic network of its kind.

100+ credentialed clinic relationships. A credentialing program that went beyond checklist compliance. $2M+ raised on vision alone. I led the commercial side — mapping the employer distribution channel through brokers, consultants, TPAs, and PBMs, and personally closing our first employer account, drafting all legal infrastructure to make that sale real.

Early-stage ventures in emerging regulatory spaces are high-variance. What this one proved: the clinic relationships can be built, the credentialing infrastructure holds, and employers will buy when the model is right and the timing catches up. That work is why founders building early-stage mental health technology — and drug companies navigating TPA and employer distribution — find their way to me.

"Nick helped translate Meeko's vision into a model the market could understand: clinically rigorous psychedelic-assisted care, delivered through a vetted nationwide ketamine provider network, and distributed through the employer benefits channel. His rare strength is moving fluently between clinical nuance, regulatory complexity, and commercial reality, then turning that complexity into practical infrastructure. For founders and leaders building in emerging healthcare categories, Nick is the kind of operator who helps make ambitious ideas credible, trusted, and executable."

SoundSelf

Clinical business development · Active

SoundSelf is one of the most genuinely novel tools in the mental health space — an immersive biofeedback system that uses voice, sound, light, and vibration to guide patients into deep meditative states, used alongside ketamine and Spravato as a preparation and integration tool. They also operate ketamine clinics directly, giving the technology a real-world clinical home.

The GTM challenge is not the technology. It is trust. Getting clinicians to change established patient care practices requires a specific kind of credibility — you have to speak their language, understand their workflow pressures, and make the case in clinical terms before commercial ones. I lead clinical business development for SoundSelf's market entry and ongoing expansion into the ketamine and Spravato clinic space: opening relationships, mapping clinic pain points, and creating the conditions for adoption in an industry where trust is the actual currency.

Embla Health

Strategic consulting · 3-month engagement

Embla is a Danish healthtech company doing something genuinely different in the GLP-1 space — behavior-first coaching paired with microdosed GLP-1s and structured tapering plans. They came to the US with $10M raised and a real problem: GLP-1s were on backorder, and American pharmaceutical procurement required a specialist.

I came in embedded with the founders and marketing team. We mapped the full procurement landscape — 340B, international sourcing, compounding, manufacturer direct, distributor partners, and payor channels — and made deliberate, defensible choices. By the end of the engagement, Embla had a durable procurement pathway built around compounding for defined medical necessity and a branded procurement strategy including international sourcing. I also identified claims in their marketing collateral that created regulatory exposure under FDA promotional guidelines — protecting the company before they became problems.

"Nick guided us through GLP-1 sourcing and business strategy with a rare mix of regulatory depth and operator pragmatism. He pressure-tested vendors, flagged both opportunities and compliance risks early, and helped us structure a resilient plan. In a chaotic market, his advice kept us moving quickly without compromising standards. He's responsive, clear, and outcome-focused — an easy recommendation."
— Nick Syhler, MD, MBA, Founder & CEO, Embla Health

Revitalist

Clinical BD & strategic advisory

When I stepped away from Meeko, Revitalist reached out almost immediately — which is how these relationships tend to work when you've built a reputation in a small ecosystem.

I came in embedded with senior leadership at one of the more established multi-location ketamine clinic networks in the country — operationally active domestically and internationally, including patient care decisions reaching the British Virgin Islands. My scope covered clinical business development, service line expansion, marketing strategy, and the talent decisions that shape an organization's trajectory. The most valuable thing a fractional operator can do is compress decision-making cycles for senior leadership — showing up with enough context to give an honest read, fast.

"Nick is the kind of person this field needs more of. He brought a solutions-focused mindset to every challenge, avoiding hand-wringing and dead ends. What set him apart was his ability to move fluidly across clinical, operational, and relational domains at a time when most people are still siloed in one. He's a genuine Swiss Army knife in the best sense: versatile, precise, and exactly what you reach for when it matters."
— Gillian Flaherty, formerly VP Operations, Revitalist

Alyea Therapeutics

Clinical & formulation consulting

Alyea is a pharmaceutical company developing Low Dose Naltrexone and a topical medication as non-addictive alternatives for Complex Regional Pain Syndrome and chronic pain, with FDA Orphan Drug Designation and late-stage clinical trials on the horizon. Their development pathway includes 505(b)(2) strategy, where real-world compounding data and clinical experience are directly relevant to regulatory submission.

What I brought was something most clinical trial teams don't have access to: hands-on compounding experience with the exact compounds in development. LDN and topical agents aren't theoretical to me, they're formulations I have worked with across hundreds to thousands of patients in clinical settings. That practitioner perspective materially changes how you think about formulation decisions, dosing rationale, and protocol design.

My scope included formulation input on a product in development, review of international clinical trial legal documents, and C-suite advisory through a period of organizational transition.

I work with companies whose mission I can stand behind. When that alignment shifts, I move on.

Stealth Telehealth Company

Embedded operator · Active

There is a version of this engagement that looks like advisory work. This is not that. I am embedded inside an early-stage telehealth company alongside medical leadership from Thirty Madison alumni, operating at the intersection of clinical, regulatory, and product in a way that most consultants cannot.

The scope spans clinical protocol development, state-by-state rollout strategy, DEA guidance and federal oversight navigation, pharmacy procurement partnerships, legal counsel relationships, and business infrastructure build.

On the clinical side specifically: I serve as Clinical Safety Advisor across pharmacy and regulatory landscape — building ketamine clinical safety architecture including titration protocols, dose ceilings, stopping rules, and lab strategy, alongside at-home care monitoring frameworks and standard-of-practice development.

On the product side: I work with the clinical team to translate medical and regulatory requirements into documentation the product team can actually build from. Bridging that translation gap is where most telehealth companies lose time and credibility. We close it early.

On the regulatory side: telehealth operates in a permanent negotiation between state licensing boards, federal DEA scheduling, and an evolving federal oversight framework that did not exist five years ago. Working alongside the team, we have mapped that architecture from the inside and built the compliance infrastructure to operate across it.

This is not an advisory role. I am part of the build.
